Compare PDF: questions worth answering

Does it compare text or pixels?
Pixels by default, because that reflects what a reader actually sees. A text-level diff is available in the summary for documents with a text layer.
Why does everything show as changed?
Usually a re-scan or a different export resolution. Normalise both through Compress at the same DPI first, then compare.
What if the page counts differ?
Ream compares the overlapping pages and lists the surplus pages as added or removed.
